The client presented for a routine visit as pleasant, well dressed, and behaved in a good manner. Has a history of long-standing bipolar disorder and was recently diagnosed with traumatic brain injury after a car accident. The client is on Depakote 750 mg once daily. We decreased the dose by tapering as follows: 750 mg x 2 weeks once daily, 500 mg x 4 weeks once daily.
